Consistent Care Services (CCS) has been a leader in Washington in addressing the needs of the underserved including the homeless, individuals with substance use disorder, and those with mental illness for the last 10 years. CCS has locations across the state of Washington including Spokane, Kennewick, Walla Walla, Yakima, and Marysville and serves a total of 11 counties. Multiple programs are offered at this time including Emergency Department Visit Reduction Program, Readmission Avoidance Program (RAP), Overdose Management, Summit Program, Treatment Recovery and Support, and Hotspotters.
At Consistent Care Services, we take a patient-centered approach to case management in response to inappropriate use of healthcare services. By using a community health model, we engage social resources within the community to collaborate on comprehensive care plans that address the health and social challenges faced by our patients. Our RN case managers and community health workers (CHW) work directly with our patients and their families to navigate the health and social care systems in an effort to improve access to care and their quality of life.
