Consistent Care Services is seeking a Spoke Care Navigator (SCN) who is empathetic, charismatic, a team-player and passionate about helping a complex and vulnerable population working for the Washington State Targeted Response (WA-STR) Hub and Spoke Project. The SCN can be a chemical dependency professional (CDP), Social Worker (SW), Licensed Practical Nurse (LPN), Medical Assistant (MA), Community Health Worker (CHW), Emergency Medical Technician (EMT), Certified Recovery Coach or Certified Peer Counselor.
 
Roles and Responsibilities
The SCN will be the Spoke’s designated staff that will work directly with patients that are interested in MAT services and will facilitate referrals to the Hub for MAT, withdrawal management services, chemical dependency counseling, and/or mental health counseling. CCS’ SCN will work very closely with area Emergency Departments (ED) and Emergency Medical Services (EMS) agencies.  The SCN may also provide case management in order to arrange primary care, coordinate employment services, arrange transportation, find housing and food resources. The SCN will provide MAT education and training for staff at the Spoke. When necessary, represent CCS in professional meetings within the community.
 
Required Qualifications
  • 2-3 years of experience in a healthcare setting
  • Ability to operate a computer, fax machine, and multi-line phone
  • Previous experience with Microsoft applications – Outlook, Excel, Word, etc.
  • Understanding of insurance eligibility and verification processes and knowledge of billing guidelines
  • Effective time management and organizational skills
  • Position requires extensive knowledge on the use of Motivational Interviewing
  • All personnel are required to maintain credentials
  • Must have a valid WA State driver’s license, car insurance, and own a vehicle

Represent Consistent Care Services in professional meetings within the community including training staff from all Spokane County Emergency Departments and all Spokane County Emergency Medical Service (EMS) providers how to refer patients with substance use disorder into a HUB for MAT.  Assist EMS providers on-scene with placing an overdose patient into MAT.
 
Required Qualifications
